Acute necrotizing encephalopathy (ANE) is a rare, potentially fatal neurological disorder predominantly affecting pediatric patients, with a poor prognosis and high mortality rate. Here, we report a case of a 22-year-old woman diagnosed with ANE, analyzing neuroimaging features correlated with clinical outcomes. Brain magnetic resonance imaging demonstrated characteristic extensive bilateral symmetric lesions involving the thalamus, external capsule, optic tracts, and mammillary bodies.
